Forms Required for New and Returning Scouts

Note: These forms must be completed at the start of each Scouting year by all Cub Scouts.

Other Useful forms

BSA Youth Application - for boys who have not yet registered with the Boy Scouts of America
BSA Adult Application - for parents who wish to become adult leaders and require registration with the Boy Scouts of America

Please bring completed forms and any required fees (check or cash only) to Pack 518’s Back to Pack event on 8/27/2011, starting at 9:00am in Shadowbrook Baptist Church’s Youth Ministries Building.

Or

Please bring completed forms and any required fees (check or cash only) to one of Pack 518’s monthly pack meetings, usually held the first Monday of each month, starting at 7:00pm in Shadowbrook Baptist Church’s Youth Ministries Building.
